Thank You Event 2020: Project of the Year - Winner
Every year we celebrate our amazing staff and their achievements at our pop-up ceremony. This year to keep everyone as safe as possible the event will be held virtually - Board members will pop into video meetings to surprise colleagues and present them with a trophy to celebrate as they are shining examples of our magnificent staff behaviours.
We will be posting all the winners as we reveal them so keep a look out on our website and social media channels.
Project of the Year - Winner
The development and mobilisation of the Leeds Virtual Frailty Ward (LVFW) is a terrific example of colleagues across the Leeds system demonstrating collaboration, commitment and determination in finding solutions for the benefit of older people living with frailty and their carers.
The Leeds Virtual Frailty Ward is a collaboration between NHS organisations in the city (primary, community and secondary care), Leeds City Council Adult Social Care and the Leeds Oak Alliance (Third Sector consortium). The virtual frailty ward provides rapid assessment (within 2 hours) and wrap-around care to people in their own home who become suddenly unwell and who would normally be admitted to hospital, or who are already under the care of Leeds Teaching Hospitals, with needs that can be safely managed at home.
Katie Smith, Business Manager, who nominated the LVFW said:
“We have a project culture of learning and listening, appreciation and support, enabling a can do attitude despite a highly pressured and political backdrop.
The team makes such a positive contribution to people’s lives, a good example is an email we received from the daughter of a LVFW patient. The daughter wanted to thank the team for the kindness and professionalism shown to her 94 year old mum. Mum was very frail and had been bed bound for several months following her last hospital admission. Over a six week period mum also had input from her GP for back pain. The daughter decided to phone the Neighbourhood Team for advice, within 40 minutes the community matron was at Mum’s and did a fantastic job. He took bloods, provided a prescription, which daughter was able to get and administer straight away and 94 year old Mum avoided a hospital admission and was able to be kept comfortable in her own home, which was her wish.
This sums up the team - all working together for the best outcome and achieving it.
Executive Director of Finance Bryan Machin virtually dropped in to their team meeting to surprise them with the good news.
